The following privacy statement should be at the beginning of all surveys created in Qualtrics.

This information is collected by St. Francis Xavier University in accordance with the provisions of the Freedom of Information and Protection of Privacy Act (FOIPOP) and the Nova Scotia Personal Information International Disclosure Protection Act (PIIDPA) concerning consent, usage, and protection. Captured data will be used to {purpose for which data collected, used, stored, disclosed, accessed}. This online service is hosted by Qualtrics, an American-owned company. While Qualtrics currently stores your data in Canada and has put measures in place to adhere to Canadian privacy legislation, the information you provide may now be subject to U.S. laws, including the U.S. Patriot Act which allows authorities access to the records of internet service providers.
